Dashboard must be p1 or p2 data AND published. It can be hosted on a UCSF Tableau server (UCSF public or UCSF Production (PRD) server) or public.tableau.com.
Note that is the responsibility of the dashboard owners to determine privacy level of the data displayed and where the data is permitted to be hosted. 

Provide your business case for publishing to this site
Provide the name of Tableau server to which dashboard is published (example: https://tableau.ucsf.edu)
Name of folder to which the dashboard was published (for internally-facing production server, this must be the UCSF Open Access Dashboards folder)
Full Name of dashboard. What you would like this dashboard called and identified on the site as.
Tableau published workbook name and sheet name (sheet name is the name of page or dashboard you wish users to land on)
Shortened dashboard name for small menu display. example: full name is ‘UCSF COVID-19 Vaccinations Dashboard’ ==> short name is ‘COVID-19 Vaccinations’
